Quatris Healthco Announces RevUp Chronic Care Management Services

April 9, 2019

Quatris Healthco, a cloud EMR and practice management solutions company that helps small practices boost financial performance and improve patient care, announced the launch of RevUp Chronic Care Management in partnership with MD Revolution. This new service efficiently and effectively engages eligible patients between office visits, empowering them to adhere to care plans and enabling coordination and compliance with provider care plans.

“We partner with providers to enhance care and increase organizational profitability,” said Quatris Healthco CEO Peter Bauerlein. “This integrated chronic care management service is specifically designed to enrich the patient-provider relationship and improve patient health. RevUp Chronic Care Management can help practices provide cost-effective, high-touch care that allows the practice to benefit from fee-for-service reimbursement, while preparing for value-based reimbursement models in the future.”

“The MD Revolution team works as a trusted extension of a practice to engage patients between office visits to achieve better health outcomes and patient satisfaction, all seamlessly integrated into their existing electronic health record,” said MD Revolution CEO Parker Hinshaw.

The Quatris Healthco RevUp Chronic Care Management platform delivers services to patients on behalf of providers and can be implemented in two to four weeks with minimal disruption to practice workflow. RevUp’s licensed healthcare professionals analyze patient behavior, engagement methods, and clinical indicators to help patients reach their goals. The web and mobile applications help patients track and monitor medications, follow up on lab tests and appointments, check blood sugar levels and adhere to prescribed exercise and nutrition programs. All engagement and activities are fully integrated with Centricity Practice Solutions’ electronic health record and practice management solutions to streamline billing and provide a comprehensive view of the patient’s health between office visits.

Medicare and Medicaid beneficiaries with multiple chronic conditions are the heaviest users of health care services. In January 2015, the Centers for Medicare and Medicaid Services (CMS) recognized the need to compensate providers for delivering non-face-to-face care management for Medicare patients with two or more chronic conditions. RevUp has demonstrated case-controlled and statistically significant improvements in clinical indicators and cost of care
